In some cases, it is important for your child to have reports that demonstrate the progress they have achieved or the plans you may have for their upcoming therapy and care. NAPA Centre has a range of reports that are able to be requested to assist you with this need.
NDIS Reports
NAPA Centre NDIS reports have been created in way to ensure it provides all the required information that your NDIS planner will be looking for. Whether you are doing your child’s first plan, annual planning, plan review or plan appeal, we work with you to ensure that the report covers the history of your child, their progress and your future goals for them.
Please consider carefully the life goals you are putting forward as part of your child’s plan, as they will be required in the report request. Our best results with NDIS reports is where you fully brief us on what you are hoping to achieve for your child so we can place as much details as possible in the reports.
Please note, these reports are complex and can take some time to prepare. Be aware of your child’s plan dates, and provide us as much notice as possible. NAPA cannot guarantee we can meet a NDIS meeting date if we are not given adequate time to prepare a report.

Intensive Summary Report
From January 2021, every intensive that is completed at NAPA will come with an Intensive Summary Report, which will be an additional charge on your Intensive invoice. This report will summarise the activities undertaken by your child during the intensive and the progress that was made against their goals. This report will automatically be sent to you within 3-6 weeks after the completion of your intensive.
Whilst the Intensive Summary Report contains useful information about your child’s progress during that specific intensive, we recommend completing a full NDIS Report for upcoming NDIS Reviews or planning meetings.
Assistive Technology Report
Assistive technology is an umbrella term that includes assistive, adaptive, and rehabilitative devices for people with disabilities, while also including the process used in selecting, locating, and using them.
Upon request, a Assistive Technology Report can be written to support you in attaining funding for equipment or technology needed to support the development of your child. We regularly complete Assistive Technology Reports for the following items:
- Walkers
- Standers
- Sit-to-Stand Systems
- Crutches
- Orthotics (AFO’s) & Supportive Shoes
- Physiotherapy tools/Equipment
- Adaptive Bikes
- Home Seating (excluding wheelchairs)
- Highchairs
- Occupational Therapy Toys, tools and equipment
- Feeding Tools
- Communication Devices
- Some Strollers and wheelchairs
- Splints, Wraps & Specialised Clothing
As part of the report fee, we will liaise with suppliers to arrange quotes and if required, equipment trials. Please note however that appointments for equipment trials must be booked separately and is not included in the Report fee.
Please note that NAPA is unable to comment on equipment we have not seen your child use or trial. NAPA is also unable to comment on equipment for vehicles, bathrooms, home modifications and certain wheelchair needs.
Due to NDIS requirements, an individual report is required for each piece of equipment.

Letter of Support for Low Cost Assistive Technology
This is a simplified letter of support for assitive technology or equipment that you may use for your child that falls both under $1500 and has been supported by your therapist for daily use and/or following of a Home Exercise Program. The letter of support is not as comprehensive as a full Assistive Technology Report (above), which may be required for more complex or higher priced items.

How much does a Report Cost?
- Please note that as of 1 Jan 2021 the following fee structure will apply to all reports:
- NDIS Reports
- * One Discipline Report $380
- * Additional Disciplines $190 each
- Assistive Technology Report (not incl Trial) $760
- Intensive Summary Report $190 (will be added to each Intensive Invoice)
- Letter of Support for Low Cost Assistive Technology $190
- Other reports $190 per hour
- Report Rush Fee $190
- Report Amendments $190 per hour
How long does a report take?
Different reports take differing amounts of time to complete and are reliant on us being able to find time in our therapists’ schedules to complete them.
Soon after we receive your Report Request, we will be in contact with you to let you know how quickly we can complete the report. In general, most reports will take around 3 working weeks to be completed, especially if there are multiple therapists who are contributing. Where possible, please be mindful of requesting your reports well in advance of when you actually need them.
We understand that sometimes you may need a report urgently or have a last minute meeting come up with the NDIS. If your report is urgent, you can also request a Report Rush Fee of $190 to be applied. Again, we cannot guarantee turnaround by your required date, but will confirm with you as soon as we can.
Can I use NDIS Funding to pay for Reports?
In general, report fees can be paid for using your NDIS Funding however this may be dependant on your child’s individual plan and how your funding is currently allocated.
Can I edit or have changes made to the report after it is completed?
We suggest that you take the time to complete the Report Request form thoroughly so that all information you would like included in the report is represented how you would like it.
Once your report is finalised and sent to you, it is not intended to be changed in any way. We will gladly correct mistakes or omissions on our part, but if changes are requested based on new information or a partially completed Report Request form, then a report amendment fee may be charged.
The information contained in all NAPA Reports contain privileged or confidential information, including patient information protected by federal and state privacy laws. It is intended only for the use by the person(s) intended. As all reports are signed off by our qualified therapists, any review or amendments must be approved by, and only be made by, members of the NAPA Centre therapy team.
